Background: Hypertension among the elderly worldwide is estimated to affect approximately 1.4 billion adults in 2024 (WHO, 2024), with an increase to 29.2% in 2025. In Indonesia, the prevalence of hypertension, according to data from the 2023 Indonesian Health Survey (Survei Kesehatan Indonesia/SKI), shows that the incidence of hypertension increases with advancing age. This case study illustrates the differences in family responses to the implementation of lifestyle modification therapy in elderly patients with hypertension.
Methods: This case study uses a comparative qualitative design with a case study approach, which aims to explore the problem and gain a deeper understanding of family nursing care for hypertensive clients with ineffective family care management through the application of Lifestyle Modification Therapy. In this case, there are 2 families that meet the inclusion and exclusion criteria, namely hypertensive clients with ineffective family health management problems. This case study involved seven weeks of family management, structured around three active home visits combined with continuous remote monitoring.
Result: Case study results revealed differences in response, with one family experiencing increased engagement and improved health behaviors, while the other family failed to achieve optimal outcomes due to less conducive psychosocial and environmental factors. These findings confirm that successful hypertension management depends not only on education but also on family support and the patient's psychological and environmental well-being.
Conclusions: Nursing interventions involving education and lifestyle modification support in two families with elderly hypertensive members demonstrated a positive impact on family health management.
